The Health Information Management (HIM) Specialist Lead collaborates with, trains and mentors the HIM Site Specialists. The HIM Specialist Lead is the subject matter expert for all site based HIM activities. S/he will be responsible for assisting the other site based Specialists with their daily duties to ensure compliance with all Westside Family Healthcare policies and procedures. S/he will also assure that medical records are gathered and processed in compliance with all applicable state and federal regulations and in accordance with HIPAA and HITECH privacy rules.

RESPONSIBILITIES
- Maintain the privacy and security of medical records in accordance with applicable state and federal regulations such as HIPAA and HITECH.
- Visit each site location at least bi-weekly to provide support and assistance to the HIM Specialists.
- Provide backfill coverage when primary site based HIM Specialist is out of the office.
- Participate with HIM Specialists, HIM Analysts, and HIM Manager in developing workflow improvements for the HIM Specialist team.
- Provide initial training for newly hired HIM Specialists as well as ongoing training reinforcement for existing team members.
- Assist HIM Manager with site audit activities and provide input on the performance of site HIM Specialists.
- Serve as subject matter expert for duties of the HIM Specialist and collaborate with other departments at the site level.
- Perform all duties of HIM Specialist
Requirements
- High school diploma or GED;
- Six months medical office support training and experience with Electronic Health Records;
- Strong working knowledge and use of computers and programs, including Microsoft Office;
- Above average awareness of HIPAA regulations;
- Must have reliable transportation and the ability to routinely travel between clinical sites.

About the Role:
1. Plan, supervise and coordinate the work activities of HR staff relating to recruitment, screening, selection, orientation, training, development, compensation, benefits, employee relations and recordkeeping to ensure compliance with applicable laws, regulations, policies and procedures and timely and accurate completion of necessary tasks
2. Analyze and modify compensation and benefits programs to establish competitive programs and help retain talent and ensure compliance with legal requirements
3. Implement integrated performance management systems to ensure employee focus on WFH goals, development of employee capabilities and alignment of reward systems and retention of talent
4. Handle complex internal employee matters, such as investigations, and administration of terminations
5. Analyze training needs in human-resources-related areas, including employee engagement and management, interviewing, performance management, documentation, organizational policies and equal employment opportunity
6. Analyze data concerning personnel-related data such as hires, transfers, performance evaluations and absenteeism rates to identify and determine causes of personnel problems and develop recommendations for improvement
7. Serve as Joint Commission Human Resources Chapter Champion and ensure that WFH complies with Joint Commission standards
8. Maintain familiarity with HRSA BPHC requirements for human resources operations in community health centers and ensure WFH’s compliance with such requirements
9. Participate in the development and implementation of WFH’s strategic plan
Requirements
1. Bachelor’s degree in Human Resources, Business Administration or Management, Communications, or related field
2. Certification by SHRM or HRCI (SHRM-CP, SHRM-SCP, SPHR)
3. Ten years of experience in the areas of talent acquisition, training, compensation and benefits, and employee relations
4. Previous experience using HR information systems (Paylocity preferred) and Microsoft office software (Outlook, Publisher, PowerPoint, Word and Excel)
5. Three years of experience in a human resources leadership role
6. Excellent written and oral communication and problem-solving abilities

RESPONSIBILITIES
- Perform comprehensive screenings and assessments of a patient’s social, emotional, environmental, financial and other support needs to identify factors influencing the patient’s well-being.
- Connect patients and families to appropriate internal services and community resources. Consult and interact with community agencies, state social service centers and school districts to identify and obtain resources for resolving patients’ health, psychosocial and /or financial problems.
- Support behavioral health integration by managing a registry of patients identified by the collaborative care team. Manage behavioral health referrals and provides case management connecting patients to the appropriate level of care for mental health and substance use disorder
- Facilitates regularly scheduled caseload consultation with the Behavioral Health team and communicate treatment recommendations to the patient’s medical provider.
- Accurately identify, address and prioritize at-risk patient populations and address crisis interventions as needed
- Assure implementation of care plans throughout the continuum of care to include a variety of settings (i.e. home, school, work site, clinic)
- Travel as needed between Westside sites to provide coverage and social service assistance as needed
- Maintain, share and apply current knowledge of available resources for durable medical equipment, inpatient and outpatient rehabilitation services, adult day care, foster care, nursing home placement, etc. and connect patients to these services as appropriate
- Maintain and apply current knowledge of relevant Medicare/Medicaid, and commercial insurance carriers, rules and regulations
- Maintain and apply current knowledge of the impact of relevant naturalization and immigration laws on applications for benefit programs.
Requirements
MINIMUM OBJECTIVE QUALIFICATIONS
- Bachelor’s degree in psychology, social work or another human services area and at least 3 years of relevant experience
- One year of experience providing services to a culturally diverse population
- Three years of experience using computers and Microsoft Office software

- Educate patients, the general public and special population groups particularly the children regarding oral health

Responsibilities of the Dental Patient Services Representative
- Greet all patients
- Distribute “New Patient” and “Health History” forms to new patients and assist with completion if needed
- Use two identifiers, name and date of birth, to verify patient identity
- Register and update current information in Dentrix, with proper spelling and verified information
- Verify patient’s insurance eligibility with the insurance carrier
- Collect co-pays and enter payment into Dentrix and provide payer with a receipt at time of transaction
- Tie out daily payments received between Daily Deposit Sheet and payments in drawer
- Diligently manage schedule, adhering to the providers’ templates
- Contact referrals to setup appointments

RESPONSIBILITIES:

What you will do:
- Conduct mental health assessments, diagnosis relevant conditions, provide patient education, formulate treatment plans, and manage acute and chronic mental health needs, including substance abuse, within Westside’s Primary Care-Behavioral Health Integration model of care.
- Consult with medical providers on appropriate therapeutic drug regimens and other treatment strategies.
- Provide appropriate psychopharmacology interventions, including prescribing medications, ordering, and interpreting diagnostic studies and lab tests and other appropriate treatment in accordance with state and federal laws and regulations and in accordance with Westside policy.
- Coordinate appropriate referrals in collaboration with multidisciplinary teams.
- Support the stabilization of patients discharged from mental health hospitalization back into primary care for follow-up and ongoing management.
- Use current evidence-based practice, research, and clinical knowledge to improve patient outcomes and reduce complications.
- Provide educational opportunities and training to Westside nurses, medical providers, and other staff, including education and precepting for students rotating at Westside.
- Abide by all organizational, state, and federal policies and regulations related to confidentiality, and patient rights.
- Maintain competence in clinical practice through continuing educational experiences and completion of required trainings to maintain licensure and certification.
- Participate in performance improvement and risk management activities, utilization review, and community education and outreach programs as requested.
- Complete other duties as assigned.

RESPONSIBILITIES:
The Physician Assistant delivers quality primary medical care in an efficient manner with emphasis on health promotion/disease prevention, collaborates with the clinical team of providers and allied health professionals and participates in the development of new programs, review of existing programs, and quality improvement activities.
- Provide primary medical care to patients, including collecting and recording patient information in the EMR in a thorough and timely manner; prescribing or administering treatment; ordering, performing, and interpreting tests and analyzing records, reports, and examination information to diagnose patients' conditions; monitoring patients' conditions and progress and reevaluating treatments as necessary; explaining procedures and discussing test results or prescribed treatments with patients; advising patients concerning diet, activity, hygiene, and disease prevention; referring patients to medical specialists or other practitioners when necessary; and participating in hospital admission and follow-up of patients, making arrangement for services as needed
- Coordinate work with other members of the care team, including nurses, social workers, rehabilitation therapists, pharmacists, psychologists, and other health care providers
- Provide emergency medical treatment to WFH patients as needed, and provide emergency medical treatment as requested or appropriate to non-WFH patients who are at or near a location where the physician is present for the purpose of providing services to WFH patients
- Participate in activities to increase appropriate utilization of available services by patients of Westside Family Healthcare, Inc.
- Participate in staff education and professional development activities, including continuing education credit requirements to maintain licensure and Basic Life Support certification
- 10. Participate in WFH performance improvement programs and in community education and outreach programs offered by Westside
Requirements
- Graduate of an accredited Physician Assistant Program
- Completion of an appropriate accredited medical residency program
- Licensure to practice medicine in Delaware
- Basic life support certification
- Ability to travel between worksites as needed
- Certification or eligibility for certification by an applicable American Medical Specialty Board
- Eligibility for staff privileges to local hospitals associate with WFH
- Full-time experience in the practice of medicine in direct primary patient care, both outpatient and inpatient

- Support telehealth visits: similar to rooming patients, the MA will prepare the patient for the provider portion of the visit, this includes identifying the chief complaint, completing a medication reconciliation, addressing care gaps, confirming the patient’s pharmacy
- Prepare exam rooms for patient exams, ensuring that exam rooms are neat, clean, well-stocked and properly prepared.
- Collect urine, blood, tissue, or other laboratory specimens, log the specimens, and prepare them for testing
- Operate electrocardiogram (EKG) or other equipment to administer routine diagnostic tests
- Prepare and administer medications and vaccines as directed by the provider with appropriate written orders
- Provide education to patients regarding treatment procedures, medications, diets, providers’ instructions and preparation necessary for diagnostic tests, x-rays and laboratory examinations
- Clean and sterilize instruments and dispose of contaminated supplies
- Perform and document point-of-care equipment testing daily before beginning patient care

MINIMUM OBJECTIVE QUALIFICATIONS
- High school diploma or GED

- Basic life support certification
- Experience using Microsoft Office software and electronic medical record software

Responsibilities of the Medical Assistant Clinical Trainer:
- Working along with the Nurse Clinical Trainer, the Associate Director of Clinical Operations, and the Director of Clinical Operations, develop evidence-based training that supports education and ongoing development of the Clinical Operations team.
- Provide training, support, and remediation as indicated to the Clinical Operations team.
- Support and/or coordinate orientation and training of new hires.
- Provide education on and implementation of workflow changes resulting from the addition or revision of policies and processes.
- Perform routine and ad hoc chart audits and direct observations of the Clinical Operations team in coordination with the Nurse Supervisors and Clinical Floor Supervisors.
- Participate in recurring meetings with the Nurse Clinical Trainer, the Associate Director of Clinical Operations, and any other meetings at the request of the Associate Director of Clinical Operations or the Director of Clinical Operations.
- Provide ambulatory care support as a member of the care team, as needed.
- Participate in quality improvement activities and initiatives.
- Other duties as assigned or requested.
Requirements
MINIMUM OBJECTIVE QUALIFICATIONS
Medical Assistant:
- High School Diploma or GED.
- Successful completion of a medical assistant training program at an accredited institution.
- Active Medical Assistant certification from AAMA, AMT, NAHP, NCCT, NCMA or NHCA.
- Minimum of three years’ experience in ambulatory care setting or comparable setting.
- Current BLS certification.

Registered Nurse
- Three years’ experience in ambulatory care setting or comparable setting.
- Active RN licensure in the State of Delaware with no restrictions or disciplinary action.

Core Responsibilities
- Schedule and monitor the performance of daily tasks by the designated MA group to ensure they are working in accordance with Westside’s standards, policies and procedures
- Supervise MA team to include performance evaluations, performance improvement plans and formal corrective actions documentation when needed
- Ensure efficient patient flow during the sessions, rooming and delegating tasks as necessary to MA team members
- Participate in the recruiting, interviewing, orientation and training of new MA team members
- Organize and lead monthly team meetings
- Assist the MA team and providers in completing tasks when necessary
Requirements
- Successful completion of a medical assistant training program at an accredited institution
- At least 5 years of experience as a medical assistant, preferably in the area of ambulatory care
- Current Medical Assistant certification from AAMA, AMT, NAHP, NCCT, NCMA or NHCA

The Ambulatory Care Nurse RN supports patients by facilitating implementation of the prescribed medical treatment plan through patient-centered education of disease processes and wellness opportunities and through patient self-management skill development.

RESPONSIBILITIES OF NURSES
- Manage a caseload of patients identified as at risk for health deterioration, sentinel events and/or poor outcomes using the nursing process and established protocols/workflows. Ongoing evaluation of care coordination efforts by measuring interventions effectiveness and fostering a continuous improvement mentality. Partners with the patient and family, the primary care provider, and other team members to implement an interdisciplinary plan of care.
- Participates in daily huddles with the Clinical Operations team, and other team meetings/trainings as scheduled
- Proactively and continuously reviews patient medical records to identify any care needs requiring intervention or follow up during patient interactions
- Works with patients and families to ensure both medical and psychosocial needs are met in order to promote health and well-being and to identify any barriers that inhibit the patient from adhering to the prescribed plan of care
- Educates patient/family about medical conditions (existing or newly diagnosed) to assist them in appropriate self-management
- Proactively incorporates lifestyle improvement opportunities and preventative care into patient interactions, empowering patients to reach their individual health goals
- Assesses and documents progress towards goals based on provider’s plan of care; utilizing clinical judgment, established protocols, review of patients’ self-monitory tools and trends in clinical data (i.e., HbA1c, LDL, BP)
- Promotes timely access to appropriate care, increasing the utilization of preventative care and healthy behaviors to improve the health of the population at risk

The Cancer Screening Patient Navigator is a contracted position funded by the Delaware Division of Public Health. The Cancer Screening Patient Navigator will guide individuals through the cancer-screening process. RESPONSIBILITIES
- Identify patients in need of cancer screening and guide them through the cancer screening process.
- Provide education and support regarding the need for cancer screening.
- Serve as an advocate for patients and providers by determining and meeting their needs as well as underscoring the importance of screening and being active participants in their own healthcare.

RESPONSIBILITIES OF NURSES
- Manage a caseload of patients identified as at risk for health deterioration, sentinel events and/or poor outcomes using the nursing process and established protocols/workflows. Ongoing evaluation of care coordination efforts by measuring interventions effectiveness and fostering a continuous improvement mentality. Partners with the patient and family, the primary care provider, and other team members to implement an interdisciplinary plan of care.

RESPONSIBILITIES
1. Supervision and HR Management:
- Directly supervise the Assistance Manager of Enrollment Services, the Community Navigator Supervisor, and the team of Enrollment Specialists.

RESPONSIBILITIES
- Identify and assist patients in need of MOUD services through outreach to connect them and to keep them engaged in MOUD treatment and/or behavioral health services as appropriate
- Collaborate and communicate progress and barriers with the PCP, Behavioral Health and the care team as needed to create and implement individualized care plans for patients
- Model competency in recovery/wellness and create a compassionate, safe environment to discuss substance abuse treatment and discuss the positive benefits of long-term recovery
- Assist patients and families with enhancement or acquisition of social and life skills, positive coping mechanisms, self-care and self-help skills
- Recognize early signs of crisis to connect patients and families to appropriate services, including access to Narcan and Narcan training
- Create and implement individual and group activities that will further enhance recovery process
- Complete social determinants of health screenings to identify social and health related needs and connect patients to internal and/or community resources
- Advocate for the social and emotional well-being of patients in recovery and their families within the healthcare system and the broader community.
- Maintain accurate and timely documentation in the electronic medical record. Document progress and treatment recommendations in EMR and the registry so that information is shared with medical providers, psychiatric consultants, and other treating providers.
- Build and maintain relationships in the community to stay informed about local resources and assist in developing support networks for patients

- Assists uninsured patients in completing a Sliding Fee Scale application, Title X scales, and/or directs them to an Enrollment Specialist when necessary
- Assists patients with scheduling follow up appointments or moves existing appointments within the schedule as needed
- Assists patients with laboratory orders, signing patients in for lab work, verifying insurances, printing orders

The School Based Social Services Coordinator is a core member of the collaborative care team who provides access to social services and resources in the community. As a School Based Social Services Social Services Coordinator, you will be instrumental in creating a nurturing and supportive environment for students and their families at East Side Charter and in the surrounding Wilmington community. This dynamic role encompasses proactive engagement with parents and the school, close collaboration with local organizations, and the provision of direct support to address social and health-related concerns for students and their families. The ideal candidate will excel in working on a multi-disciplinary team, ensuring meticulous documentation, and serving as a pivotal link between the school system and the broader community.

What this role does:
- Establish and nurture relationships with EastSide families and staff through active participation in parent events, staff meetings, and community gatherings.
- Facilitate workshops, events, and informational sessions to promote health and wellness awareness.
- Collaborate with local organizations to enhance community support services in the Wilmington community where EastSide Charter families live.
- Provide direct support to students and their families, addressing social and health-related concerns.
- Perform comprehensive screenings and assessments of a patient’s social, emotional, environmental, financial, and other support needs to identify factors influencing the student’s well-being.
- Collaborate with the interdisciplinary team to create and implement individualized care plans.
- Assist families in navigating and accessing available community resources, social services, and healthcare facilities.
- Stay informed about local resources and maintain an up-to-date database for easy reference.
- Advocate for the social and emotional well-being of students and their families within the school system and the broader community.
- Work collaboratively with EastSide Charter staff to address social issues impacting student success.
- Occasionally support the operational needs of the School-Based Health Center that may include patient registration, schedule appointments, and other duties as needed.
- Maintain accurate and timely documentation in the electronic medical record. Document progress and treatment recommendations in EMR and the registry so that information is shared with medical providers, psychiatric consultants, and other treating providers.
- Perform other duties as assigned.
Requirements
- High School Diploma or General Equivalency Diploma (G.E.D.)
- Strong written and oral communication skills
- Self-starter with the capability to work independently and collaboratively.
- One year of experience providing services to a culturally diverse population
- Three years of experience using computers and Microsoft Office software
- Ability to perform duties directly in the communities as needed (up to 90% of the schedule will be required to work in communities throughout Delaware)
- Verifiable good driving record and reliable transportation
- Ability to work evenings and weekends as needed.

Westside Family Healthcare is a nonprofit organization that provides high quality primary medical care without regard to ability to pay. The Enrollment Specialists work to remove financial barriers to healthcare by connecting uninsured people, including Westside Family Healthcare (WFH) patients, to programs that make healthcare more affordable. In addition to providing education about affordable health care coverage options through one-on-one interactions and outreach events, Enrollment Specialists provide assistance with the application and enrollment process for State and Federal insurance affordability programs. These programs include Medicaid, CHIP, the Health Insurance Marketplace, Health Care Connection (HCC) and Screening for Life (SFL). Additionally, Enrollment Specialists assist WFH patients with enrollment in our sliding fee scales and external medication assistance programs. Strong organizational skills are required.

What we expect from you:
- Provide assistance to WFH patients and consumers who need access to financial support for health care services, including the Health Insurance Marketplace
- Help individuals understand program eligibility criteria and the application process for various financial assistance programs that help to make health care more affordable
- Explain other health programs that increase health care access for which the patient or consumer may qualify and connect them with resources for program enrollment
- Assist with program application completion, identifying and obtaining required documentation and serve as a liaison with the program administrator to complete the enrollment process
- Successfully complete Certified Application Counselor or Navigator certification during the initial training period and maintain annually thereafter
- Complete timely, detailed reports for all patient/consumer activities and outreach events
- Protect patient and consumer information to ensure that confidentiality is maintained in accordance with HIPAA regulations
- Travel to and participate in statewide outreach events as needed, typically 2-4 times per month
- Attend all required meetings and trainings
- Develop extensive knowledge of health care and social service resources available in the community
- Provide information on the importance of primary and preventive care during one-on-one interactions and at community outreach events throughout the state
Requirements
- High school diploma or GED
- Six months or more of experience using Microsoft Office software suite (Word, Excel, Outlook)
- Fluency in written and spoken English and Spanish or English and another language spoken by a majority of our patient population
- Valid driver’s license, reliable transportation and ability to travel independently between work locations, including external outreach events
